Invoice requirements

Health 

  • The total amount due must match the loan amount
  • The product or procedure the customer purchased must be documented

Home Services

A Home Services invoice must contain the following information:

  • Customer name and address
  • Address of where the product is delivered and installed
  • Itemized description of purchases with corresponding prices
  • The total amount due must match the loan amount

 

Payment information requirements

Void personal check

The check must be:

  • Associated with your checking or savings account
  • Printed, not hand-written (this includes the name that appears on the check)
  • Legible and complete (not cut-off)

Bank-provided form

  • Must be complete with account information
  • If the form is hand-written, it must contain a bank stamp

 

Accepted forms of photo ID

  • Driver’s license
  • State ID
  • Passport
  • Armed Forces ID
  • United States Alien Registration Card (Green Card)

  

Accepted Proof of Income (POI) types

Pay stub

The pay stub cannot be handwritten and must be dated within 30 days of the application date.

Note: The application assessment is based on regular hourly earnings. Borrowers with non-regular pay must provide a federal income tax return or W-2 in addition to a pay stub in order to have those earnings considered.

Federal income tax return and W-2

If a significant part of your income is based on non-regular pay (for example, bonuses, overtime, part-time, seasonal), or if you are self-employed, you can submit your most recent federal income tax return or W-2.

If you receive pension, government allowance, social security, or disability payments, you can submit your most recent federal income tax return or W-2.

Note: Federal income tax returns and W-2 forms must be submitted in addition to a pay stub.

Bank statements

If you receive pension, government allowance, social security, or disability payments, you can submit bank statements for the three most recent months. Bank statements must show consistent deposits in each month.
